96 mustang 3.8 v6 Auto Transmission gets stuck inbetween gears?

I have a 96 stang and most of the time it shifts great. Some times it will get stuck in a low gear and not shift (I stop at 4000 RPM) Sometimes it shifts but then the engine rev's but its like it in neutral and the car is just coasting. Anyone have any idea's
